Fantastic - that's solved a 5 year problem! I'd never noticed that setting.
Might it be worth adding a comment to the options page highlighting this?
Is it possible to override the GUI setting with a command line option?

The command line is
"C:\Program Files\ExamDiff Pro\ExamDiff.exe" "old.txt" "new.txt" /i /w
and here's a screenshot showing the result on a file with whitespace and case differences (plus the Ignore options that are set in the GUI)
